In 1998 the world health organisation (WHO) estimated that despite all the efforts to eradicate measles it still accounted for more than 30 million infections and 1 million deaths every year (2). This organism is an RNA virus of the genus morbillivirus, hence measles sometimes being referred to as morbilli. This virus belongs to the virus family of the Paramyxoviridae (4). The measles virus is transmitted through bodily fluids mainly as aerosols (airborne exposure) or droplets (5). It enters the host through the respiratory tract and immediately starts to replicate in the epithelial cells of the respiratory tract (6), from here the virus start to invade some cells of the immune system in the lymph nodes particularly the monocytes (7) through which it then spreads to rest of the host body. Measles is a self limiting disease, which means it will normally resolve itself after a few weeks, but because measles also induces a transient profound immunosupression (8), most of its victims succumb to fatal opportunistic infections. The level of measles virus in the plasma is plotted versus day of infection (7) It should be noted that in very rare cases the measles virus cannot be cleared from the host and persist in the host system in what is termed persistent measles virus (PMV). This is the cause of most measles complications which includes a subacute sclerosing pan encephalitis (SSPE) (5)(9) which may occur in about 1:10 000 measles cases (9) and inclusion encephalitis (5) which may occur if the host does not have adequate cellular response to the infection. Infection and Spread Clinical symptoms of measles include fever, malaise, coryza (runny nose), conjunctivitis and tracheobronchitis (10). Other symptoms that appear at a later stage during infection are the Koplik’s spots, 10-12 days post exposure, and erythmatous maculopapular rash which appears at around day 14 (5). Symptoms like diarrhoea and pneumonia, which are from opportunistic infections, will not be discussed here as they are not a direct result of the measles virus. The direct results will be discussed later in this essay after discussing how the virus infects and spread around the host body. To fully understand the mechanism of measles virus infection, one has to understand the measles virus structure. The virus As a morbillivirus in the paramyxoviridae family, the measles virus is a negative, single strand enveloped RNA virus (11). It is about 150-300 nm in diameter and has a lipid bilayer surrounding the RNA forming the envelope. Protruding from this envelope are two glycoprotein ligands called haemagglutinin (H) and fusion (F), necessary for virus attachment to host cells. The RNA is enclosed in a nucleoprotein (N) and associated with two proteins called phosphoprotein (P) and large protein (L) (12). See diagram below for a schematic picture of measles virus. (5) The lipid envelope on the outside of the virus is acquired from the host cell during budding (13), because this outer layer is made up of host material it becomes difficult for the body’s immune system to detect the virus as non-self in the initial stages of infection enabling the virus to gain a foothold. Directly beneath the lipid envelop is the matrix (M), a protein that is important in virus replication as it facilitates the assembly of virus particles to the cell surface membrane during budding (5). The nucleoprotein (N) forms a protective sheath around the virus RNA called the nucleocapsid. The associated proteins P and L have been suggested to act as virus polymerase by some studies (12), helping in RNA replication. Then there are the two glycoproteins H and F. As their names suggest, they are responsible for anchoring the virus to the host cell and penetration of the cell membrane. The haemagglutinin binds (agglutinates) the virus to the host cell receptor (CD46, to be discussed later) and the fusion protein fuses the virus envelope with the cell membrane (5). Infection The infective process occurs in two key stages of attachment and fusion. In order for the virus genome to gain entry into the host cell and start replicating it needs to â€Å"catch† and â€Å"inject† host cells with its genome. The â€Å"catching† of host cells occurs when the virus haemagglutinin protein (H) attaches to the host cell receptor CD46. CD46 in the human cell act as a co-factor for serine protease degradation of C3b and C4b (14), these are the complement proteins of the immune system hence immunosupression by the measles virus. Studies (15) suggest that there exist more undefined virus receptors that enable the virus to attach to its host cells. The formation of syncytiae, cell-to-cell contact, also allows virus to spread to other cells (5). The â€Å"injecting† of host cells with the virus genome occurs during fusion, and the processes are less understood. It is thought that the same fusion processes occur for all enveloped virus, and studies (16) on human immune-deficiency virus (HIV) has shed some light into this. Two glycoproteins are involved, gp41 and gp120. gp41 is anchored onto the virus envelope and gp120 attaches to the host cell CD4 receptor next to the CCR5 co-receptor. Structural changes then occur whereby the gp41 is driven into the membrane of the host cell thereby linking the two cells (virus and host). The whole structure of gp41, gp120, co-rector CCR5 and CD4 receptor then collapses leaving the two membranes in apposition for fusion (16). Spread As discussed earlier, the virus enters the host through the respiratory tract, where it replicated in the epithelial cells. Then it infects cells of the immune system, monocytes are the mostly affected cells (6,7). Symptoms The symptoms of measles infections as mentioned earlier are fever, malaise, coryza, conjunctivitis, cough and Koplik’s spots in the oral cavity, later on the erythmatous maculopapular rash appears. These symptoms are tied with the immune’s response as the virus infection progresses. From day 0 when the virus first enters the host to around day 8, the first response from the immune system is the innate one, which is characterised by inflammation, giving rise to fever and general body malaise. As the virus continues to replicate in the epithelial cell of the host especially the nasal cavity (causing coryza), the trachea and bronchus (causing coughs), the nasolacrimal duct (causing conjunctivitis) and the oral epithelia cells (causing Koplik’s spots), it irritates these mucosal surfaces and cause their inflammation. The appearance of Koplik’s spots is diagnostic of measles (7) and it signals the appearance of early symptoms and viral load start to peak in th e blood (refer to fig 1 for the measles timeline in-host infection). These spots are transient and last for only about 3 to 4 days; also it is about this time that the adaptive immune response starts to kick in. At day 14 the viral load is at its peak and the adaptive immune response is fully operational and is clearing the pathogen from the system, the formation of the immune complex on the skin as the virus gets cleared result in the measles rash signalling cytotoxic T cell clearance of virus-infected cells (5, 7). After peak levels of the virus, following successful adaptive immune response, the virus levels drop and the immunological memory begins (7). In rare circumstances the virus can persist in the host, causing complications (17). One of these complications is the subacute sclerosing panencephalitis (SSPE), discussed below. Subacute sclerosing panencephalitis This is one of the complications of persistent measles virus infection; it affects the central nervous system of children who are infected by measles virus at a very early age (18). These children normally lack the maternal antibodies in their circulation to help combat the infection at its early stages. Studies (19) found that 50-75% of children who develop SSPE had measles infection before the age of two and also the prevalence rates of SSPE are 1:10 000 cases (9). This complication can take a very long time to manifest itself, with an average time of 8 years before SSPE symptoms appear and the reported range is between 9 months and 30 years (9). The disease leads to neurological deficits and eventually the patient dies (20). The other two measles complication worth mentioning are progressive measles inclusion body encephalitis (MIBE) which can occur in patients who are immunocompromised, and the post infection encephalomyelitis which is an autoimmune disorder that attacks the myel in sheath covering neurons. Recovery from measles (assuming opportunistic infections have not prompted medical intervention) requires both the humoral and cell mediated immune response. Cytotoxic T lymphocytes clear infected host cells and measles antibodies reduce free viral load by serum neutralisation (5) these antibodies can also be directed directly against the virus. The humoral response as usual is mostly for preventing re-infection and is involved in building immunity, a process taken advantage of by vaccine developers. After cytotoxic clearance of infected cells; there follows an immunologic type 2dominance where cytokines IL4, IL5 and IL10 are secreted by type 2 CD4+ T cells (21). Vaccination Many virus infections are untreatable, they are either self limiting, maim, or kill the host, so the only intervention that is available is to prevent infection in the first place. As noted from fig 1, after day 21 when the pathogen has been cleared, immunological memory is activated. This activation of immunological memory can only happen after the host has encountered the measles virus. Subsequent infections are met with a robust humoral response and the virus does not progress to cause infection. This is the basis of immunisation, the host immune system has to be introduced to the virus first, and as there is a risk of the virus overwhelming the immune system most virus used in vaccines are â€Å"controlled†. Attenuated virus vaccine Once the measles virus was isolated and cultivated by Enders and Peebles (5) it was then able to be manipulated. They found that the virus once cultured in chick embryos was attenuated in virulence and immunogenic enough to stimulate the host immune system to produce antibodies against it and remembering it (22). Work to produce a licensed measles vaccine begun in 1958 (5) and by 1963 the first licensed measles vaccine, RUBEOVAX ®, was available. In 1968 a more attenuated vaccine ATTENUVAX ® was licensed. As more vaccines for viruses e.g. mumps and rubella were developed there was a need to combine the doses into one single shot. In 1971 (5) a trivalent vaccine with all the three vaccines, measles-mumps-rubella was licensed. In recent times a fourth vaccine has been added to the MMR vaccine, resulting in a tetravalent vaccine MMRV (23). The fourth vaccine is a chicken pox vaccine, varicella. Other virus vaccines Other less favourable virus vaccines exist. Experiments were done on high titre vaccines by Sabin et al (5). These were aimed at infants who are at risk because they have circulating maternal measles antibodies which prevent vaccine uptake by neutralising it. The other vaccine, an inactivated measles virus vaccine produced by killing the virus in formalin was produced and licensed in 1963. It fell out of favour because the immunity if offered lasted for only one year and it had to be taken in three doses (5). With vaccination and great knowledge gained from studying measles it can be said that total eradication of the virus is possible. A Refreshing Analysis of T.S. Eliot’s â€Å"The Love Song of J. Alfred Prufrock† To say that The Love Song of J. Alfred Prufrock is a typical romantic ode to the wonders of love, as the title may suggest, is quite far from the truth. To the contrary, this poem enters the straggling mind of J. Alfred Prufrock, a man plagued with irresolution, and because of this irresolution will probably never realistically be in love with a woman. Love Song is a dive into Prufrocks inconsistent thought processes, and the foggy workings of his less-than-optimistic mind. Through bleak imagery, a wavering tone that feels timeless, and carefully connoted diction, T.S. Eliot portrays J. Alfred Prufrock as an uneasy, indecisive, and ultimately scared man. The first few lines of the poem set the scene as to what kind of content Prufrock has to offer. He uses a simile in comparing the evening, spread out against the sky, to a patient etherized upon a table (2-3). Its a fairly unappealing comparison, and it puts an awkward image in the readers mind from the beginning. He goes on to set the scene of a kind of tour through a city-like atmosphere: Let us go, through certain half-deserted streetsof restless nights in one-night cheap hotels and sawdust restaurants (4-7). Again, a bleak image is cast into the mind of the reader, reminiscent of a twisted Gotham City where no one would want to be unless accompanied by someone very dear a someone who Prufrock is not with. He goes on to make another type of etherized comparison later on, which adds to the bleak, uneasy feeling: And I have known the eyes already, known them all- The eyes that fix you in a formulated phrase, And when I am formulated, sprawling on a pin, When I am pinned and wriggling on the wall, Then how should I begin To spit out all the butt-ends of my days and ways? And how should I presume? This stanza dives into Prufrocks uneasy nature through the use of carefully connoted diction (e.g., pin) that conveys imagery of a bug or animal ready to be dissected. He is describing what happens when he runs into people he may know: they fix [him] in a formulated phrase, and then pin him down, and when he is pinned and wriggling on the wall, he is then forced to interact with them and make a decision about how to go about saying how his day went. To almost everyone else in the world, this type of interaction is a daily occurrence of life, and usually isnt conveyed as a feeling of being pinned down. However, Prufrocks uneasy nature is very similar to that of The Catcher in the Ryes Holden Caulfield, in the fact that he really isnt one for lighthearted social interaction. Instead, every little detail of life, in Prufrocks eyes, is not considered an idle task, but a high-strung, uneasy chain of decisions. Throughout the poem, Prufrock seems to jog around in his mind, and is quite abstract with his thoughts. The result is a wavering, fragmented tone that further suggests Prufrocks indecisiveness and digressive habits. One of the subtle ways that Eliot adds to this wavering tone is the fact that no definite rhyme scheme is used throughout Love Song. For example, one stanza includes mostly rhyming words, ending lines with words such as dare, stair, and hair, and then thin, chin, and pin. But the next couple of lines in the stanza may have no rhyme pattern at all, and the same goes for the next stanza; its totally fragmented. This wavering rhyme scheme cleverly adds to the notion of an indecisive Prufrock. Besides the wavering rhyme scheme, the overall tone suggests that Prufrock is very uneasy and indecisive. Prufrock really does continually ask questions, always questioning things. This may seem normal, but considering the subject matter and the uneasy feeling connoted with them, this mode of thought does not come off as entirely healthy. There are close to 20 stanzas in Love Song, and in almost all of them, Prufrock is questioning something. Whether the subject matter consists of whether he should disturb the universe or not, or how he should deal with people who ask him how his day is, he is constantly questioning everything. He almost mockingly asserts his indecisive manners by saying Do I dare disturb the universe? In a minute there is time for decisions and revisions which a minute will reverse (45-48). Essentially, he is declaring that he can make a decision now, with only a minute left, but since in one minute there will be no time anyway, hes leaving it at that. This feeling of time passing too rapidly is present throughout the poem. More than a couple of times he says, And indeed there will be time, or a variation of this line, which not only adds to his irresolute manner, but also reiterates the fact that he often trails off and picks up another topic on a whim again, recalling Holden Caulfields digressive tendencies. Prufrock even directly refers to this tendency when he analyzes a womans arm in the lamplight; he says, It is perfume from a dress that makes me so digress? (65-66). He mentions, too, his awareness of the passage of time and of the fact that he is growing old by confirming that he is becoming slightly bald. Ultimately, this realization of mortality makes him afraid: And in short, I was afraid (86). Towards the end of the poem, the tone of Love Song seems to waver more and more, and Prufrock becomes even more of a shaky, uneasy, scared figure. There were rough times as well as good times. If I were to tell you all of them, I would remember half of them. I think some of my incidents really had some impact, and some were just simple ways of life. To tell you the truth, the incident that had the most impact on me has to be when my real father left me at the age of three. I never knew my father. I mean being a baby, you really have no experience or recognition of somebody else. My mother got pregnant at eighteen years of age. From what I was told, she knew my real father well enough that she wanted a child from him. As a result, she gave birth to me in April of 1981. She was so happy and glad for having her first child. My mother informed me that in the beginning she and my father lived well and tried their best to raise me. As time passed, my father was always drunk and cared less about anything. If I was sick, he never went out to buy me medicine or took care of me. He would just be drinking and complain about everything. My mother realized she was tired of him. He wasn’t good for anything. As my mother told me, he’d come from work and get into arguments and sometimes beat up on her. When it came to that point of physical abuse, she had enough of him. One day, he got into a big fight with my mom. He nearly destroyed the apartment where we lived. That night, he went to a bar and got drunk. My mother told him to never come back. Days passed and he never returned. H... ... my mother never mentioned my real father again. It’s like when we began talking about him, we just talked about the coward he was. In retrospect, now that I matured, I really don’t think about my real dad. He was just a stranger to me. I’m very proud of myself for being the type of person I wanted. An ingrained expectation of culture places certain constraints, most pertinently on the female role, as both authors openly explore the issues faced by those with a basic inability to cope with such pressures; ultimately leading to their mental downfall. Neither Nicole nor Esther fit the social order required of them causing others to label them as insane, but it is the negative influence of society upon them which is the overwhelming cause. Within ‘TisN' the reader witnesses Nicole Diver's fundamental need for a male presence in which contrasts with the strong feminist beliefs of Esther Greenwood. Both novels clearly demonstrate how the masculine world dictates that women are to be regarded as possessions and constantly manipulated by a male counterpart – be it their father or husband. The lack of a father in Nicole's life allowed Dick Diver to become both a replacement father figure and husband giving him escalated dominance which ultimately caused the pressure on Nicole as an individual to increase significantly. Nicole's world had shattered, ‘but it was only a flimsy and scarcely created world' due to the incestuous relationship she was a part of. These flaws in Nicole's upbringing resulted in her inability to create a life for herself as the past still troubled her. Nicole and Esther mutually place men on a pedestal not only due to their reliance on them but also the ideals society forces upon them in regards to male superiority; until they realise that not only can Dick and Buddy not live up to these expectations, but also that they are unrealistic for the society that they live in. Living in a patriarchal society makes Nicole dependent on a controlling figure to make ethical judgements for her. She waits for ‘Dick to make a moral comment', rather than continuing to develop the individuality she was free to express in her private letters to Dick when explaining the mental health problems she was experiencing at that time. Similarly, when Buddy, who has never skied himself, ‘instructs' Esther in the sport, she mindlessly obeys. ‘TisN' Book 1 shows a complete contrast as Nicole is described as ‘happy to exist in a man's world' referring to her allowing a husband or lover to take charge, portrayed with both Dick and Tommy. Nicole is trapped in her feminine role in the 1930s as, although she feels that Tommy ‘opens up whole new worlds for her', he is simply a new domineering character in her life who ‘waited five years' for her marriage to be over. Contrastingly, Esther is able to find power on the slopes which then allows her to see through the hypocrisy of society in making men appear to be the superior gender; mirroring the counter culture revolution emerging in America at the time. This also challenges the mentality of women regarding sexual equality and control as Esther's disgust mounts over Buddy ‘having an affair with that tarty waitress' while continuing to expect a virgin bride. The limited sexual choices available to Esther cause her to view the world as ‘divided into people who had slept with somebody and people who hadn't', thus making us aware of the importance of sex significantly a lack of for women, within this community. Nicole also has a breakthrough in exerting her independence and forcing Dick to take responsibility, when she realises that he ‘is a coward! who tries to blame his failures on her. This disappointment in realising that Dick is not what she had anticipated allows Nicole to see past the disillusionment. This independence is strengthened as the book ends from Nicole's viewpoint as Dick ‘became a dot' in her life. This allows her to become a dominant character which was a great accomplishment before the feminis t movements of the 1960s. Society isn't the only trigger however and the disillusionment of the protagonists is an underlying cause of their mental decline. Nicole uses her relationship with Dick as an escape from her illness but when family life becomes mundane it resurfaces. She consciously uses her marriage to evade problems however she was soon proven wrong as her marriage only instigated more demands on her character due to the expectations of a wife, apparent not only in terms of the society the book was set in, but also that of the writers' reality. Scott Fitzgerald according to Horace Gregory of the New York Herald Tribune, was said to be ‘sharing the failure of his protagonists' as the life of Nicole closely mirrored that of his wife Zelda. This allowed him to use his experiences regarding the effect of society's pressures, and reflect them in Nicole Diver's life. Her dreams of love and accomplishment are quickly distorted by the possession of property which causes emptiness in her life of important things – despite many viewing her life as perfect. Similarly, Esther primarily idolises Buddy as superficially he appears to be perfect: a handsome Yale student with an interest in her despite being older, though soon she finds that ‘he had fooled [her] all these years' with hypocritical actions that society had allowed. 930's American Society had one expectation of women – to marry and have children. Nicole initially gives the impression that she has easily fallen into this position however the role is forced upon her as she struggles to establish autonomy. The demands on Nicole's character similarly build up as she is pushed into becoming wife and mother, while keeping to the traditions of the Warren family. From afar, Nicole appears to have the perfect life, the grand-daughter of a horse trader with ‘white crooks eyes', nevertheless all the riches of the world could not stop her breakdowns. It is apparent that Nicole wants nothing more than to lead a happy life with her family, however being part of the idle American expatriate community, a worldly attitude towards life easily takes over since Nicole has ‘too much money'. This money-orientated outlook on life even goes as far as to drive her to use money as an advantage in her relationship with Dick even though ‘he did not want to be owned', showing that despite the facade of having everything, she was neither happy nor would she ever be under such pretences. Equally Esther's inability to earn her desired place at university is an example of Sylvia Plath attacking the philosophy which gives false hopes to many – especially the youth. These values also feature in Esther's move to New York as despite any outsider believing she was ‘having the time of her life' the truth of the situation couldn't differ more. She ‘was supposed to be the envy of thousands of college girls' however she found the materialistic flaws in society too much to deal with and conventional expectations to be cold after such anticipation. Nicole's anticipation for love and accomplishment mirrors this. In conclusion, society is seen to be the contributing factor towards both Nicole and Esther's mental declines as they both strive to accomplish all that they can. It is once reaching these aims that they realise its irrelevance and it is this disappointment which ultimately becomes too much for each protagonist to deal with.
